Long-Term Care: 

Needed when your loved one is at a point in life when they need assistance with activities of daily living. Fall Creek provides rehabilitative and ongoing skilled nursing care to residents.

Short-Term Care: 

Sometimes your loved one needs a place for a short time. Normally this is several weeks or a few months following a surgery, injury, illness, or other medical condition.

Respite Care:

When the primary care giver needs time away for their own needs, Respite Care steps in to fill the gap. After a short 3-7 day stay in our facility your loved one can head back home.

24-Hour Licensed Nursing Care: 

Nursing services made up of talented and compassionate, Registered Nurses (RN) and Licensed Vocational Nurses (LVN) that are always on hand, 24 hours a day, 7 days a week.

Hospice Services: 

Our care team provides end of life medical, psychological, and spiritual support. The goal of hospice is to provide peace and comfort to an individual and their family.

Skilled Nursing Care: 

When a resident's need for care or treatment can only be done by licensed nurses such as wound dressings, rehabilitation, or rapidly changing health status, we have the correct skill set on site.

Rehabilitation and Therapy:

After a stroke, surgery, injury, or illness, our rehabilitation team will create a customized program tailored to meet your goals. We offer rehabilitation services 7 days a week and provide physical, occupational, and speech therapy. A detailed list of our extensive rehabilitation equipment can be found here.

Support Services on Site:

We want to take care of your overall well-being. When you need additional help, we will support and assist you. Some support services we offer are Audiology, Advanced Wound Care, Cardiology, Dialysis Transportation, Registered Dietitian, Podiatry, Psychiatry, Pulmonology, Discharge Planning, Hospice, Laboratory, 24/7 Access to Doctor, X-Ray Services, Ophthalmology Pharmacist, PM&R Physician, and Pharmacy.
